Pick the kind of claim. Each one goes to a different partner, so starting in the right place saves days.

Staff injury (workers' comp)

  1. 1The employee tells their supervisor or HR right away.
  2. 2If your district uses Company Nurse, the employee calls them for triage.
  3. 3HR completes the accident report and gives the employee the DWC-1 claim form.
  4. 4HR files with Keenan through Ultra ClaimsONLINE, with the report, DWC-1 and salary history.

Property damage or liability

Damage to district property, or a claim that the district caused harm. These go to ASCIP, the property and liability partner.

  1. 1Make the area safe and take photos.
  2. 2Write down what happened, when and who saw it.
  3. 3Report it to ASCIP with the photos and notes.

Student accident

An injury to a student during school or a school activity. Complete the student accident report and send it to the district office.
